DENTAL INSURANCE

We accept a wide variety of dental insurance policies. With verification of insurance and assignment of benefits by the insured, we will file claims on behalf of our patients in exchange for payment for services provided. Dental insurance is a contract between an employee, an employer, and an insurer and does not relieve responsibility to our office for charges incurred.

As a result of previous patient care, our software database has dental insurance information for hundreds of local, national, and global companies. It is very likely that one of your co-workers has been seen by Dr. Tallis for dental treatment.

If you would like to see if your dental insurance can be used through our office, simply give us a call. We will be happy to verify your benefits prior to making your appointment. If you prefer, you can contact us through the website with this request, and submit a "Dental Insurance Confirmation" form from the PATIENT FORMS page.

Patients with verified dental insurance are required to pay estimated deductibles and copayments at the time of treatment. All patients are provided with written estimates of these charges upon initial consultation. Regardless of dental insurance coverage, patients with insurance remain responsible for payment of all incurred charges.
